Caloocan asks A3 cluster vaccines: Bring valid ID, proof of comorbidity
THE Caloocan City Government issued a notice to A3 priority list category or those 18-59 years old with any controlled comorbidity to bring valid identification card and proof of comorbidity when going to COVID-19 vaccination center.
Mayor Oscar “Oca” Malapitan said any of the following may be provided as proof of comorbidity issued within the past 18 months:
Medical certificate from an attending physician, medicine prescription for the past six months, hospital records such as discharge summary and medical abstract and surgical records and pathology reports.
“Tandaan lang na basta nakapag-profiling o registration na ay maaari ng magtungo sa vaccination site na malapit sa inyong lugar. Hindi na po kailangang maghintay ng text message para sa appointment,” Mayor Oca said.
